Black Jack doesn't sentimentalize suffering, it just shows you what happens when a man who operates outside the system becomes the only person willing to save you. The two stories here strip away the procedural comfort of earlier episodes, one tears into Pinoko's history, the other throws Black Jack into a conflict where his skills become a weapon in someone else's war. Black Jack returns in two separate stories in this dramatic OVA sequel to the 1993 release. In the first, Pinoko is struck by unexplained attacks of pain that tests cannot diagnose, and the case leads Black Jack to Yuri Saionji, a former patient he was once briefly involved with. Helping her may also be the way to help Pinoko, and old feelings resurface. In the second, Black Jack is kidnapped on his way home by plane and taken to a war-torn country ruled by a totalitarian dictator. The dictator has a brain tumor that seems impossible to operate on, and he demands that Black Jack remove it without anesthesia while a resistance movement prepares to rise up.
