Germany panics over a ring he gave Italy and can't figure out why everyone keeps winking at him. The entire OVA is built around watching the most uptight character in the series slowly realize what he's accidentally suggested to every nation with eyes. It's comedy that works because the punchline lives in the gap between what Germany thinks is happening and what literally everyone else already knows.

Hetalia: The Beautiful World OVA collects four comedy extras that were released with the home video volumes of The Beautiful World. The first two, Buon San Valentino parts one and two, have Germany becoming comically muddled about where he stands with Italy, and a ring shaped like a tomato plays a part in the mix-up. The other two episodes are The Centennial Gift and Liechtenstein and the Hedgehog Country.
